Clinical studies that have lowered the percentage of dietary carbohydrate and/or the glycerin index of the carbohydrate have consistently shown improvements in glycerin control among individuals with type 2 diabetes [4-8]. Lifestyle modification using low-carbohydrate diet interventions are effective for improving obesity and type 2 diabetes, and may play an important role in reversing the current epidemic of ‘diabesity. While this study was a treatment trial of individuals with type 2 diabetes, lifestyle modification has been shown to prevent type 2 diabetes in the Diabetes Prevention Program (DPP). Alzheimer’s disease, atherosclerosis, diabetes and even scoping and cancer, just to mention a few have an important inflammatory component, though disease progression seems also dependent on the genetic background of individuals.
This strong affairs between dietary carbohydrate and postprandial serum glucose led to the development of medications that block carbohydrate absorption for the treatment of type 2 diabetes [3].
The diagnosis of other types of diabetes is usually made in other ways. Transplants of exogenous beta cells have been performed experimentally in both mice and humans, but this measure is not yet practical in regular clinical practice partly due to the limited number of beta cell donors. Children with antibodies to beta cell proteins (ie at early stages of an immune reaction to them) but no overt diabetes, and treated with vitamin B-3 (niacin), had less than half the diabetes onset incidence in a 7-year time span as did the general population, and an even lower incidence relative to those with antibodies as above, but who received no vitamin B3. Wolfram’s syndrome is an autosomal recessive neurodegenerative disorder that first becomes evident in childhood.
Microscopic or nanotechnological approaches are under investigation as well, in one proposed case with implanted stores of insulin metered out by a rapid response valve sensitive to blood glucose levels.
The body’s cells become progressively dehydrated as water is taken from them and excreted. The fact that 40% of diabetics who carefully control their blood sugar nevertheless develop neuropathy, and that some of those with good blood sugar control still develop nephropathy, requires explanation. Various mechanisms are proposed, including alterations to the vascular supply of the brain and the interaction of insulin with the brain itself. Prolonged high blood glucose causes glucose absorption, which leads to changes in the shape of the lenses of the eyes, resulting in vision changes; sustained sensible glucose control usually returns the lens to its original shape. This increases the osmotic pressure of the urine and inhibits reabsorption of water by the kidney, resulting in increased urine production (polyuria) and increased fluid loss.
